Refill Your Prescription
Prescription refills can be handled by your pharmacy. Please contact your pharmacy directly for refills. To expedite handling your request, have your pharmacy fax any medical authorizations requests to our office, during regular business hours. Our fax number is (602) 956-7591. Medications prescribed for your current infertility treatment cycle are reviewed and handled at each of your appointments.
