Making Your Appointment

We encourage both partners to be present at your initial infertility consultation.  If at all possible keep this in mind when actually scheduling your initial infertility consultation.  You may contact our office at (602) 956-7481 Monday - Friday 8:30am to 4:30pm to schedule your first appointment.   Alternatively, use the contact form on our website to request request an appointment and we wil confirm with you by phone.  Be sure to include the best time to reach you, so that we may accomodate the sensitive nature of discussing your fertility.

What to Prepare

To make your initial infertility consultation appointment, please have your insurance information and appointment calendar(s) available.   Once your appointment is made, we will send you a letter reminding you of your appointment date and time, as well as your infertility registration paperwork, medical record release forms, and infertility medical history questionnaire for both you and your partner to complete.

After Your Initial Infertility Consultation Is Scheduled

It is important that we receive previous medical records for all infertility/gynecology work-ups on both you and your partner.  Please complete the Record Release Authorization form(s) and forward them to the doctor(s) who maintain your Infertility records.  Results of ultrasounds, labs, Laparoscopies, surgeries, current pap smear and Hysterosalpingogram (HSG) should be forwarded before your appointment date.  HSG X-ray films need to be requested from the facility or radiologist where they were done and delivered before your appointment or hand carried to your appointment.

You may hand-carry or mail your infertility / gynecological medical records to our Phoenix Office or you may have it faxed to us at (602) 956-7591. Be sure to order your medical history(s) early.  We would appreciate receiving this information at our office no later than two weeks prior to your appointment.

Insurance Coverage for Your Appointment:

Insurance coverage for infertility testing and treatment can be very confusing and frustrating. Our billing manager has extensive experience with insurance companies and will contact your insurance for you to determine your coverage.   If you have an HMO or other Managed Care insurance plan requiring authorization for a specialist visit, you must obtain a referral/authorization prior to your first visit with our office. Please contact your Primary Care Physician to obtain this referral.  As your appointment approaches our Billing Manager may contact you with additional information regarding your specific infertility level of coverage.
